Agreement between Club and Personal Trainer

I was wondering if you have an agreement or sample(s) of agreements between a club and a PT. Also if you can give an idea of standard structure for the way an outside PT might work within a club. The idea of sharing a percentage with a club or paying a monthly fee are two considerations. If a percentage is shared - what is a good rule of thumb to start with.
