They aren't really coming to learn how to get in shape, they are coming to lay their emotional baggage in you. In any way you allow them to in the name of "fitness".
Of course a good trainer listens to their client, understands that they are stressed.
I'm talking about a nightmare of a client who may have a fitness goal but comes up with every excuse under the sun including emotionally harassing the trainer to avoid their own incompetency.
Fitness is tough sometimes, a trainer can help you get comfortable with that side of it, but in the end the client must step up to the plate.
