Personal Training Health Questionnaire

The second of our business forms is the personal training health questionnaire. The main purpose of this form is to assess your clients health. You need to know whether your client has any immediate concerns that may affect their program. The personal training health questionnaire is not a full medical history. You should also ask about:

  • Any Surgeries your client may have had
  • Broken bones
  • Any joint or muscle pain

Sometimes clients can forget things when filling out the personal training health questionnaire. Its your job as the trainer to ask about each body part. For example, your client may not think they have a problems with their knees but still experience a little pain when climbing stairs occasionally. So Why Bother?

The personal training health questionnaire is designed to limit your liability. You and your insurance company don't want to be liable if your client gets injured because they didn't disclose a medical condition. While the personal training health questionnaire is not a waiver or liability, it does document the clients current medical condition.

What if a Client Discloses a Medical Condition?

If the client answers Yes to any of the questions on the personal training health questionnaire, you have a responsibility to ensure that you design a safe training program.

You would be negligent (in other words...IN BIG TROUBLE) if you design a program that aggravates an injury that your client has disclosed to you.

So make sure you investigate the injury and even speak to your client's doctor for advice. You may want to get your client's doctor to sign a medical release form allowing the client to start training. This is another great way to protect your business.
