Physicians Consent
Physicians Consent- I am a recently certified personal trainer - I have picked up several clients and I have them fill out the: Informed consent for testing and programing, AHA Health and Screening Questionnaire , Agreement and release of liability, and the Par-Q. I am starting a spring Group Boot Camp and I have been using the same paperwork- My question is...on the AHA Questionnaire It is VERY common to have 2 or more checks in the second portion of the form (Cardiovascular Risk Factors) because 2 of the questions are You do not know your Blood Pressure and You do not know your cholesterol level- add that with a very healthy 23yr old who smokes and you have 3 checks- Should I have the client get a physicians consent to protect myself legally or is it a judgment call? Or am I doing to much paperwork for a Group class? --Thanks!
