Do you charge extra for designing workouts for your clients?

Quick snapshot of my business, I left a big box gym in November of 2014 to start my own business. We offer mobile physical therapy and mobile and in studio personal training. As the trainer I specialize in 55+ corrective exercise, and functional one to one and small group training. I would love your opinion on pricing of my packages. I currently sell my sessions in packages. Five 45 minute small group sessions for $100. (4-5 members per class) Twelve 45 minute sessions for $540. and twenty four 45 minute one to one sessions for $840. In home sessions are priced higher for travel time.
I design programs to meet client's goals. I use a subscription to a workout builder program that I can design exercise programs that include tips and video for each exercise. Side note, this subscription comes with a yearly fee for me but has been a life saver! They are digital and can also be printed out from pdf format.
Client's ask me from time to time if I would send them the digital programs that I design so they can perform the workouts outside of our sessions which is a good thing, but here's where I need your opinion.... would you charge for the exercise programs?
Would you charge two different package prices, one package just training and one package including both training and exercise programs at a higher price?
Also, would you put an expiration date on your training contracts to keep cancellations down to a minimum?
