Personal Training Agreement Form

Welcome to my pages covering personal trainer forms. We will talk about each form starting with the personal training agreement form.

I have seen trainers get carried away with forms. Don't make your prospective clients feel like they are applying for a bank loan by dumping too much paperwork on them.

An easy way to do this is to use our Complete Training System.

Whenever you add a new client to your training system, you have the option to send them the new client paperwork. The client can complete all the forms in advance by logging in to a web portal.

The secret is to get what you need with the least hassle for your client.

 

The First Must-Have Form

The personal training agreement form is simply the agreement between you and your client. And you can define the agreement in any way that you want. If you want to agree with your client on topics like meeting locations, equipment rental and dress code, the personal training agreement form is the way to do it.

Cancellation Policy

This is a critical part of your personal training agreement form. You define what happens if you client cancels or doesn't show up. In the sample below we have a strict cancellation policy. However, notice that there is a "Get out of Jail free" clause to show your clients that you are not the enemy.

Here is an excerpt of the personal training agreement form from our personal training starter kit:

 

Cancellation Policy

All cancellations must be received at least 12 hours before your training session in order to avoid being charged for your session. Clients who do not cancel with 12 hours notice will be charged for the cancelled session.

<<your business name>> understands that emergencies happen. We provide every client with one free short-notice cancellation. You will not be charged for your first cancellation with less than 12 hour notice. Subsequent short-notice cancellations will be charged for the session. The free short-notice cancellation only applies if <<your business name>> is notified prior to the session start time. No shows are not eligible for the free cancellation.

The Refund Policy

If you are going to sell training packages which require your clients to pay for multiple sessions in advance, you are going to need a refund policy in your personal training agreement formpersonal training agreement form. The refund policy gives your client the assurance that they only have to pay for what they use. If you provide deep discounts for large packages, you may wish to add a small penalty that recovers the discount that was given on the used sessions.

Refund Policy

<<your business name>> strives to provide the best possible service to our clients. If for any reason you are not satisfied with our services, we will be happy to issue you a refund for services not performed.

If you have paid for a package in full, you will be refunded for unused sessions and services.

Don't Forget the Important Part!

Make sure you have your client sign and date the personal training agreement form. Its also a good idea to have them print their name since most signatures are illegible. And then file the form in your client's file so you can pull it out if you ever need to remind the client of your agreement.
